37 hours per week based across the East Riding (fixed term contract to 30 April 2020) £15,796 per annum (from 01 April)
The post holder will work as part of a team of 4 people and in partnership with NHS Community Link Workers on the Social Prescribing contract. Using your knowledge of local service provision, health needs and issues across the East Riding you will empower, motivate and support individuals to access community resources to improve their health and wellbeing. Good interpersonal skills will be key to the success of this role, you should also be competent in the use of IT systems. This is an ideal role for someone who wants to develop their experience of working in mental health. Applicants must have a full current driving licence and access to a motor vehicle for work purposes as travel across the area will be required. The post is subject to an Enhanced DBS Disclosure
