Harrogate & Rural Alliance Director

Deadline 17th March

Salary: Circa £68k

Location: Harrogate

The Alliance are looking for a Director to lead the development and delivery of a newly integrated community health and social care service in the Harrogate area.
Prevention is at the heart of this agenda, delaying and reducing demand on more acute services, building provision on the premise that prevention is better than cure.
The programme will place the person and community at the centre of everything they do, based on a strength-based approach, ensuring a service that is owned by the community and colleagues and delivers the best outcomes possible for those they support.

As the Alliance Director, you will have responsibility for overseeing the delivery of the integrated community health and social care service, organised around primary care practices and local communities, which responds to the CCG vision for community services in Harrogate and District ‘Your Community, Your Care’ as well as to the other national and local drivers within the NHS and social care.
The post holder is responsible for the provision of integrated, preventative, safe, caring, high quality, person-centred health and social care at the right time in the right place, and to support people in the Harrogate and Rural District to live independent and healthy lives.

The key objective will be to implement and embed a service model establishing hubs in defined localities which comprise multi-disciplinary teams based around primary care practices; supported by a number of health, social care, voluntary and independent sector and wider public services.

You will work closely with Harrogate and District Foundation Trust and North Yorkshire County Council and will collaborate and influence at all levels across the health and social care system and with partners. Partnership working will be fundamental to the success of this role. You will work closely with all primary care practices to ensure that the service is community and population based and puts the person at the heart of the service.

You will provide leadership and a clear strategic direction to ensure the service prospers.
