Hours: 32 hours per week. Core service hours are 9.00-5.00pm Monday to Friday, with some evening and occasional weekends dependent on client need and on call rota.
Grade: NJC Scale PO2 points 36 to 38
Funding: Currently the CfH service is currently funded until the end of August 2019
Salary: £32,233 – £34,106 pro rata, actual salary £27,877 – £29,497
Base: Beeston and other Leeds Mind offices

Leeds Mind is the lead agency, in partnership with Hamara, Touchstone, Barca-Leeds, Better Leeds Communities and Leeds Irish Health and Homes, responsible for delivering the social prescribing service, Connect for Health. The service is commissioned by Leeds South and East Clinical Commissioning Group for the people of South and East Leeds. Connect for Health has been operating since November 2015.
Connect for Health works with clients with a broad range of wellbeing and social needs engaging them with services and community groups appropriate to their needs. The Connect for Health team consists of 12 full time equivalent Wellbeing Coordinators, two Team Leaders, a Data Officer and Receptionist.
The service is well into its third year and following successful independent interim evaluation, is focussed on continuing to deliver a progressive service to reach all communities in Leeds South and East and promote social prescribing.
The post-holder will have overall service and line management responsibility of Connect for Health, ensuring effective high quality operational delivery which meets the needs of local communities and contractual requirements. The successful candidate will have an excellent understanding of health inequalities and working with marginalised communities. You will need to be experienced in managing staff and relationship management with key stakeholders. The postholder will be a highly motivated and dynamic manager who can work under their own initiative.
Being a car driver is highly desirable due to significant travel involved.
